If the Host phys were disabled because of an invalid transition, remove the SAS cable on the disabled phys completely from both IOMs and then recable correctly. Removing the cable completely allows both IOMs to reset. For other disabled host phy conditions, replace the shelf module to which the affected host phy belongs.
